A National Standard of Care to Treat the Condition of Poverty
Transition to Success® (TTS) is a standard of care to treat the condition of poverty. TTS responds to the social determinants of health by coordinating care across healthcare, human service, government, faith-based organization, and education. By maximizing existing funding streams and integrating uniform protocols based upon best practices, we can improve health and self-sufficiency.
TTS is a scalable, sustainable, multi-generational, measurable, generative business model. TTS curriculums define specific treatment protocols with a 19 domain reliability tested tool that assesses and measures outcomes related to social determinants affecting heath, education and economic self-sufficiency. TTS trained direct care staff work with clients to identify, prioritize and ensure access to all available services and supports across their community. CARE* management of existing resources integrated with peer mentoring, financial literacy and volunteerism empowers clients to achieve educational and economic success for themselves and their families.
Using a collective impact social enterprise model, the TTS community based lead organization, champions TTS & training across their community. This TTS trained CARE* Network brings consistency and improves access to all available programs.. TTS is based on a continuous quality improvement model with uniform standards of practice, creating consistency in care, measurement and evaluation.
TTS trains trainers, a model that creates a generative social enterprise to drive community based transformational change.
TTS is gaining national attention, momentum and statistically significant results.
Today TTS pilots are being led in Michigan, Tennessee and Hawaii. Transition To Success®, identified as a Clinton Global Initiative, establishes for the first time industry protocols and reliability tested measures that are continually refined based on research and evaluation to treat the condition of poverty.
